Life Decisions Program Base Units

To request a presentation, scroll down and identify the unit closest to your school/organization’s location. Under the Presentation Request Form column, double-click on the "Presentation Request Form" link in the region for the unit you have identified. Complete the "Life Decisions Program Request/Acknowledgement" form by typing in all information in the grey shaded areas. Once the information has been entered on the form, print the form and have the administrator of the school/organization sign the form. The form should then be faxed or mailed to the appropriate unit selected to conduct the presentation.

Note: Please read the form in its entirety. The request form must be completed, typed, and received 30 days prior to the requested presentation date with school/organization administrator’s signature. Request forms received after the required timeframe and incomplete may prevent the program from being scheduled.
