I have referred the above named person for a position with the Texas Department of Criminal Justice. I understand that I will receive a $100 gift card and eight hours of administrative leave for an applicant referral that results in a new hire that completes 6 calendar months of employment. I acknowledge that the value of the gift card is considered taxable income and can choose from the following options: Gift Card:

Referral must be submitted the same day or before the online application is submitted. Ineligibility includes any referrals submitted after an application has been submitted.
If unable to locate employee or applicant based on submitted information, transaction will be void.
Referred employee must be hired within six months of submission, employee is able to resubmit information if referred employee re-applies.
Without a valid e-mail address, the gift card will sent to the postal address on file with TDCJ personnel records.
